Lesson Format & Payment Preferences Note: All regular lessons are conducted online unless otherwise arranged and approved by the instructor. In-person lessons may be offered occasionally at the discretion of Musician’s Toolshed. I understand this authorization remains in effect until I notify Musician’s Toolshed in writing of any changes or cancellations. I agree to the ACH Debit Authorization terms above. Media Release & Showcase Participation Participation in online or in-person student showcases requires a media release. These events may be hosted by the instructor or held at public venues, with details provided in advance. Participation is always optional. For students under 18, a parent or guardian must be present. Additional guidelines are outlined in the Lesson Policy. By selecting an option below, you indicate your consent or refusal regarding media use: I grant permission for Musician’s Toolshed to use audio, video, or photos of me/my child for showcase, educational, or promotional use on musicianstoolshed.com and related platforms. This may include YouTube or other public platforms where showcase videos are shared using first name and last initial only.I DO NOT GRANT PERMISSION and understand that this excludes me/my child from participating in online and in-person student showcases. Acknowledgments & Agreements The acknowledgments below are required to participate in lessons. Please read carefully and confirm. 1. Lesson Policy Acknowledgment I have read and agree to the Musician's Toolshed Lesson Policy, including expectations around scheduling, cancellations, make-ups, and communication. I understand that my weekly lesson time is reserved for me and that make-ups are not guaranteed. Alternate resources may be offered at the instructor’s discretion for absences with 24+ hours’ notice. 2. Communication & Site Access I understand that enrollment includes access to a private student profile and the Lesson Dashboard on the Musician’s Toolshed website. I will be automatically subscribed to the required lesson newsletter and agree to stay up to date with lesson communications and respond when needed. 3. Technology & Connectivity I understand that I am responsible for ensuring a stable internet connection and functioning device for online lessons. Musician’s Toolshed is not liable for shortened or missed lessons due to client-side technical issues. 4. Recording Consent I understand that lessons may occasionally be recorded for quality or educational purposes. These recordings will not be shared publicly without additional consent. Students may record lessons for personal practice only. 5. Materials & Lesson Resources Parents/guardians (or adult students) are responsible for purchasing any lesson materials required by the instructor, including but not limited to sheet music, lesson books, or instrument accessories. Musician’s Toolshed will provide guidance on required items and recommended vendors. 6. Intellectual Property All materials and lesson content provided by Musician’s Toolshed are for personal educational use only and may not be copied, distributed, or republished without written permission. 7. Limitation of Liability To the extent permitted by law, I agree that Musician’s Toolshed is not liable for any incidental or consequential damages arising from participation in music lessons, whether online or in person. 8. Health & Safety (for in-person lessons) On rare occasions when an in-person lesson is offered by the instructor, I agree not to attend if I or my child is ill, contagious, or experiencing symptoms. I will follow any health and safety guidelines provided by Musician’s Toolshed.
